Steps to Request the Removal of Information from Your Background Report
Under the Fair Credit Reporting Act (FCRA), individuals seeking to block specific information from their background report must complete the following steps:
-
Verify Your Identity
To process your request, we will need to verify your identity. This can be completed in one of two ways: - Match with Your Consumer File - Provide your full legal name (first, middle, last, and suffix if applicable), any former names, and either your complete Social Security number or date of birth.
- Additional Identity Documentation - Include copies of government-issued identification (e.g., driver’s license, passport), recent utility bills, or other proof of identity, which may involve answering security questions specific to you.
-
Submit Documentation Confirming Human Trafficking Victim Status
Acceptable documentation includes a statement from one of the following sources: - A determination from a government entity, such as a Federal, State, or Tribal authority.
- A statement from a non-governmental member of a human trafficking task force, including victim service providers authorized by Federal, State, or Tribal government entities.
- A self-attestation by the individual identifying as a victim of human trafficking, which must be signed or certified by a Federal, State, or Tribal government entity, a court, or an authorized representative of these entities.
-
Identify Specific Information to be Blocked
Clearly identify the items in your background report that you believe should be removed due to their connection to human trafficking. Providing this information helps us block or review only the necessary items.
